Major Features:

  • Arterial Phase Hyperenhancement (APHE): A lesion that enhances more than the surrounding liver parenchyma during the arterial phase of contrast enhancement. This is a hallmark feature of HCC.
  • Washout: A lesion that shows less enhancement than the surrounding liver parenchyma on delayed phases (e.g., portal venous or delayed phase) compared to the arterial phase. This indicates that the contrast is leaving the lesion faster than the surrounding liver.
  • Enhancing Capsule: A smooth, measurable rim of enhancement surrounding a lesion on portal venous or delayed phases.
  • Size: The largest dimension of the lesion. Size is a crucial factor, with larger lesions generally having a higher probability of being HCC.

Ancillary Features (Not included in this simplified calculator):

These are additional features that can modify the probability of HCC, either favoring or disfavoring it. Examples include mild-moderate T2 hyperintensity, restricted diffusion, fat in a mass, mosaic architecture, and nodule-in-nodule appearance.
